Before your first appointment, we will email you a personalized link to a confidential patient portal. Once logged in, you can complete the intake forms (no printing required!) before your session. This will prevent the use of limited session time to complete paperwork. When you arrive at Flourish Louisville, please be seated in our comfortable waiting room and your clinician will meet you there at your scheduled session time.
