FEES & INSURANCE
Treatment fees are simple at Flourish Louisville.
All sessions are 50 minutes in length and you will be provided current hourly rates before you schedule. Outside-of-session services such as phone calls, consultations with your medical team, document preparation and engagement with attorneys on your behalf are billed at the same prorated, hourly rate. Payment is due in full at the time of service. Cash, checks, credit cards and HSA accounts are accepted. Most patients pay by credit card for convenience.
Flourish Louisville is not a participating provider with Medicare, Medicaid or private insurance companies. You may be able to file for reimbursement from your insurance company if your benefits plan offers coverage for out-of-network providers. You are encouraged to contact your insurance company prior to your first visit to learn about your coverage.
You may find it helpful to ask them the following questions:

• Do I have out-of-network mental health benefits?
• Are services rendered by a licensed psychologist covered?
• Is any pre-approval required before obtaining out-of-network mental health services in order to
be reimbursed?
• If I have out-of-network benefits, will I be reimbursed the full amount I paid or a portion?
• Do I have a deductible and if so, what is it?
• What documentation needs to be submitted for reimbursement?
